Personal Injury/Motor Vehicle Accidents
We only work with Motor Vehicle Accident clients who have an attorney to navigate the process and ensure payment of medical claims.
All patients that have been involved in a motor vehicle accident causing personal injury will be asked to fill out the necessary documents regarding their injury. Patients are responsible for providing the necessary insurance information that pertains to the injury (insurance company name, address, adjuster’s name, telephone number, and claim number, police reports, a letter of protection, etc). Our office will verify through the insurance carrier if the claim has been reported.
All patients that have been involved in a work-related injury will be asked to fill out the necessary documents regarding their injury. They will also need to have a referral and the visits will need to be approved by their adjuster before they are seen in our office for care. Patients are responsible for providing the necessary insurance information that pertains to the injury (insurance company name, address, adjuster’s name, telephone number, and claim number). Our office will bill the insurance company directly for all services rendered.
